If you feel sick after eating food ordered through DelivrMe, please send an email to firstname.lastname@example.org with details about the order, and answer a few quick questions to investigate.
Of course, we will need consent to pass on your details if the case needs to be investigated further.
The details required are as follows:
- What symptoms did you have?
- After consuming your order, have you been sick or unwell?
- What was it in your order that made you feel this way?
- Have you had to seek medical attention or visit a doctor? Have you been prescribed medication?
- Are you the only one to be affected or have others too?
